Terms and Conditions

You understand that you are submitting an application for dental or vision insurance marketed by Association Member Benefits Advisors. Each application includes a one-time $20 application fee that is assessed on the same day as your first initial premium (void where prohibited). You understand that if you have any further questions you can reach Association Member Benefits Advisors at (281) 317-7318. Should you decide to terminate your coverage during the first thirty days you are entitled to a refund of your premiums. You will return any claims paid during that time to the insurer. Terminations must be submitted in writing. You understand that by completing this form you are requesting coverage for the endorsed plans marketed through Association Member Benefits Advisors (AMBA).
